AmeriHealth is proud to offer a Post-Acute Care Program designed to improve recovery outcomes for our Medicare Advantage members after a hospital stay. This program brings together predictive analytics, case management, and care coordination to support members during this critical time.
- Predictive analytics help us identify members who may need extra support after discharge. By looking at their health history and their hospital records, we can spot potential risks early and intervene before issues like medication confusion or weakness turn into bigger problems.
- Case management gives members a trusted point of contact during recovery. Case managers help explain next steps, answer questions, and support members as they adjust to care at home. For older adults, this guidance can make recovery feel less solitary and overwhelming and more manageable.
- Care coordination helps keep everyone on the same page. After a hospital stay, care often involves multiple doctors and services. Care coordination helps ensure that information is shared, medications are reviewed, and follow‑up care happens as planned.
Together, these supports help older adults recover safely, avoid unnecessary setbacks, and feel more confident as they heal at home.

AmeriHealth offers PPO Medicare Advantage plans with a Medicare contract. Enrollment in AmeriHealth PPO Medicare Advantage plans depends on contract renewal.
